"yes i agree it is efficient to work that way but it may not bring out the best efficiency.

Alex, that's the reason most of the companies make a list of hierarchy, within the employees. To whom who will report withy well define responsibilities.

@ FLYINGSCOT, I totally agree with you on that. That aspect is often overlooked for some reasons. But it is a general knowledge that it is what you appreciate that does not depreciate eventually.

A flat structure can work if everyone is self-aware & lacks jealousy. If either of those are deviod, it can be disastrous.

"in most cases, waiting for a manager to give detailed instructions would not achieve the best team efficiency. "

Alex, I prefer somebody to me as a boss and direct the team with ideas and how to work. Personally believes that there should be some managers for guidance; rather than many self active managers.

I would humbly like to add one more thing and that is to remember to praise team members for a job well done and to celebrate small and large successes.  This works wonders for team spirit and motivation.

"I have heard about the term "self-organizing team" or a team without actual an "manager"? In  some cases, this works quite well."

Alex, I personally feel that such voluntary initiations can mess up everything. Too many bosses and leaders, who doesn't want to listen to others

Frank, all the points are good at ideal situation. Eventhough in all the companies most of the points are taken care; coordination won't be there. Sinking with the pears and team is important.
