From my experience of a small writer making reviews on paperial I must say that there exists a huge problem in terms of internal communication in companies. Huge corporations employ hundreds of people who sit on the hierarchical shape of a pyramide. So usually it's close to impossible to transfer the idea from a creator to an executant without any transformations.
